Transcript or Records Request

If you are a Spencerport graduate or former student, planning to further your career and are in need of a transcript of your high school records, please print and complete the PDF form linked below.

We can also fill records requests for the following:

  • Immunizations
  • Special Education Documents (IEPs/504s)
  • Verification Letters (proof of attendance, enrollment and/or graduation)

 

NOTE: Official Transcripts CANNOT be emailed to you. If you need an official transcript sent, it can be done 2 ways:

  1. Emailed or mailed directly to the college or organization (please include their contact info)
  2. Mailed to you in a sealed & stamped envelope. Once it is opened, it is no longer considered official.

 

Please print and complete the PDF form below, making sure to indicate what documents you need. This form can be emailed, mailed or faxed to us; our contact information is noted on the form.

These requests can take up to 5 business days to complete, and will be filled in the order they are received.

Transcripts/Records are FREE.
